Children new to Hazel Community Primary School:
When your child arrives at Hazel Community Primary School, we will ask you to complete various consent forms. These will pertain to how, when and where your child's data can be used in school. These consent forms remain in effect for the whole time that your child remains at our school.
You can withdraw from any of these agreements by completing a withdrawal of consent form which is available from the school office.
